Essential exchanges approximately 50% of your calculated plasma volume in about two hours. Complete exchanges 100% of it in two to four hours. Advanced exchanges 150% — 1.5× your calculated plasma volume — in a single extended session of approximately three to six hours, which raises clearance of plasma-resident substances from roughly 63% for a single-volume exchange to approximately 75–80%. Advanced is a physician-recommended option for specific clinical situations, not a default. All three are calculated from your body weight and hematocrit rather than a flat liter count, and all three are delivered to the same clinical standards — two nurses, protocol design by our board-certified nephrologist, pre-procedure lab review, post-session IV support, and ASFA monitoring. The difference is intent: Essential is built for wellness optimization, maintenance, and first-time TPE patients; Complete is the foundation of our initial reset series and intensive protocols, and remains our standard recommendation.
Advanced is a 1.5× calculated plasma volume exchange — our maximum-clearance protocol. Clearance of plasma-resident substances isn't linear with volume: a single-volume exchange clears roughly 63%, and extending to 1.5 volumes raises that to approximately 75–80%. Advanced is not our default recommendation — Complete remains the standard for most protocols. Advanced is a physician-recommended option for specific situations where a higher clearance target is clinically appropriate: higher circulating burden, or protocols where your physician determines the additional clearance justifies the longer session and larger albumin volume. It's a consultation decision, not a menu upsell.
Essential is a calculated protocol, not a flat-volume shortcut — the exchange is derived from your body weight and hematocrit, so a patient of any size receives the same proportional exchange. It is designed for wellness optimization and maintenance goals, and research suggests reducing circulating plasma burden may support those aims. For intensive therapeutic protocols — brain health, post-viral recovery, autoimmune optimization, or a higher toxic burden — Complete remains our recommendation, and your consultation is where that call gets made honestly.

Because most of what a plasma exchange costs doesn't scale with volume. Staffing, the apheresis equipment, physician oversight, and the majority of the disposables are fixed regardless of how much plasma is exchanged. The same logic runs the other direction — it's why Advanced costs less than 1.5× Complete. We price the procedure, not the liters.
Most patients begin with a series of 3–6 treatments, based on protocol. Initial series are typically built on Complete; many patients then move to an Essential Maintenance Membership (six treatments, roughly every other month) or Annual Membership (twelve treatments, monthly) to hold results. Advanced series are structured individually by our physician. The exact number depends on your health goals, current status, and how you respond to treatment. This is determined during your consultation and reviewed as treatment progresses.
A PlasmaRestore™ Essential treatment takes approximately 2 hours. A Complete treatment takes approximately 2–4 hours, and an Advanced treatment approximately 3–6 hours, depending on your plasma volume and the specific protocol. During this time, blood is continuously circulated through the apheresis machine. Post-session IV support is included in every PlasmaRestore™ protocol to support recovery before you leave the clinic.

Therapeutic plasma exchange has a well-established safety profile and has been performed in hospital and clinical settings for decades. Common side effects are mild and may include temporary lightheadedness or fatigue following the session. At Avinity, every session is overseen by trained RNs with a physician consultant available throughout. All candidates are medically screened before treatment begins.

For most outpatient PlasmaRestore™ patients, access is through a standard IV line in each arm placed by the nursing team — no surgical catheter. One line connects to the apheresis machine; the other is a smaller line used to give calcium during the session. Access is assessed at your consultation.
Yes — you’re reclined and awake for the full session, and most patients watch TV or use their phone. After the two needle sticks that get you set up, you mostly don’t feel the exchange itself. Some patients notice brief tingling in the lips or fingers from the citrate anticoagulant, which the team addresses easily; the main challenge is sitting still.
Many patients can, but for a first session it’s smart to arrange a ride until you know how you personally respond.
