PlasmaRestore™ Protocol

How Plasmapheresis Works

This process directly changes what is circulating in your bloodstream.

  1. Blood is withdrawn

Blood circulates from your body through an apheresis system.

  2. Plasma is separated

The machine separates plasma from your blood cells.

  3. A calculated amount of plasma is removed

The treatment removes a percentage of your calculated plasma volume along with the substances circulating within it — inflammatory proteins, immune factors, toxins, and circulating debris.

  4. Blood cells are returned

Your blood cells are returned with clean replacement fluid (typically albumin).

How much plasma is exchanged is the difference between the three PlasmaRestore™ protocols — 50%, 100%, or 150% of your calculated plasma volume.

At Avinity Health, plasma exchange therapy is delivered through the proprietary PlasmaRestore™ TPE Protocol, which integrates physician oversight, structured treatment delivery, and post-treatment monitoring.

Why Patients Explore TPE

What Brings People to Therapeutic Plasma Exchange?

TPE removes and replaces a portion of the plasma circulating in your bloodstream, reducing plasma-resident substances in a single treatment. Patients come to Avinity with different health goals, symptoms, and clinical histories. Your consultation determines whether TPE is appropriate and which PlasmaRestore™ protocol makes sense for you.

Chronic inflammation

Inflammatory proteins circulate in plasma and drive how you feel and function. PlasmaRestore™ protocols are structured around reducing that circulating burden, with lab monitoring tracking progress at each stage. Protocol tier determined at consultation.

Longevity & biological aging

Age-associated proteins accumulate in plasma over time and are linked to accelerated biological aging. Published research has shown measurable changes in markers linked to biological aging after a series of TPE treatments. Typically a Complete series, with Essential maintenance after.

Brain health & memory

The AMBAR trial demonstrated that plasma exchange may slow cognitive and functional decline in Alzheimer’s disease, with separate research examining its potential role in reducing neuroinflammation. Brain health protocols are structured on the Complete protocol and evaluated individually.

Post-viral recovery

Post-viral symptoms are an active area of TPE research. Candidacy is evaluated individually during your consultation.

Immune regulation

TPE can remove specific antibodies and immune-related proteins, which may help stabilize immune response patterns.

Heart & cholesterol health

TPE can reduce certain circulating cholesterol particles and lipids linked to cardiovascular risk.

Toxin & microplastic burden

Your plasma carries toxins, inflammatory proteins, and metabolic waste that accumulate over time; TPE removes a portion of that burden. On microplastics specifically, the first human study found meaningful reduction only above a baseline level — read our review of the evidence.

Wellness & maintenance

For patients past an initial series — or easing in for the first time — the Essential protocol supports a regular maintenance cadence set with the medical team.

Not sure whether TPE fits your goals?

Every PlasmaRestore™ treatment begins with a physician-guided clinical assessment to determine whether TPE is appropriate and which protocol fits your health history and objectives.

The Evidence

Potential Benefits & What the Research Shows

Therapeutic plasma exchange has an immediate, measurable effect on the composition of circulating plasma. Its clinical value depends on what is being targeted and why the treatment is being performed.

Therapeutic plasma exchange machine at Avinity Health

Removal of circulating substances

Established mechanism. Your blood carries toxins, inflammatory proteins, and metabolic waste that accumulate over time. TPE removes a portion of this plasma — along with antibodies, immune factors, and other plasma-resident compounds — potentially reducing that overall burden.

Immune-mediated conditions

Established role in specific physician-directed indications. TPE can remove specific antibodies and immune-related proteins, which may help stabilize immune response patterns.

Cardiometabolic markers

TPE can reduce certain circulating cholesterol particles and lipids linked to cardiovascular risk.

Healthy-aging research

Emerging research, not established outcomes. Research has shown measurable changes in markers linked to biological aging after a series of TPE treatments. Clinical trials, including research in Alzheimer’s disease, have shown that plasma exchange may slow cognitive and functional decline.

Why Choose Avinity for Therapeutic Plasma Exchange in Scottsdale

Where you undergo plasmapheresis matters.

At Avinity Health, plasma exchange is delivered through the PlasmaRestore™ Protocol, shaped by over 20 years of clinical experience in apheresis-based therapies. This experience informs how each treatment is assessed, performed, and supported.

The protocol is designed around the full treatment journey — from initial evaluation to post-treatment recovery — not just the procedure itself. Plasma exchange is carried out with the discipline of a hospital-based procedure within a private clinical setting, in a physician-led medical clinic — not a med spa, not an IV lounge.

Frequently Asked Questions

How much does therapeutic plasma exchange cost?

Individual PlasmaRestore™ treatments range from $5,995 to $14,995 depending on the percentage of your calculated plasma volume exchanged, and membership pricing begins at $4,500 per treatment. Every package and membership price is published in full on ourTPE pricing and packages page, and our cost guide explains how much therapeutic plasma exchange costs— plasma volume, nurse staffing, albumin, and lab work — plus the questions to ask any clinic before you book.

What is Therapeutic Plasma Exchange?

TPE is a medical procedure that filters your blood by separating plasma from blood cells. The plasma containing inflammatory proteins, immune factors, and circulating debris is removed and replaced with albumin. It is delivered at Avinity Health through the PlasmaRestore™ Protocol under physician oversight.

What is plasma exchange used to treat?

In hospital medicine, plasma exchange is an established treatment for specific autoimmune and neurological conditions such as myasthenia gravis, Guillain-Barré syndrome, and certain blood disorders. In a longevity and optimization context, patients pursue TPE for chronic inflammation, biological aging markers, brain health and memory, immune regulation, cardiovascular and cholesterol health, post-viral recovery, and toxin and microplastic burden. Which applications are appropriate for you is a clinical decision made at your consultation. For the studies behind each application, see our TPE research hub.

What is the difference between PlasmaRestore™ Essential, Complete, and Advanced?

Essential exchanges approximately 50% of your calculated plasma volume in about two hours. Complete exchanges 100% of it in two to four hours. Advanced exchanges 150% — 1.5× your calculated plasma volume — in a single extended session of approximately three to six hours, which raises clearance of plasma-resident substances from roughly 63% for a single-volume exchange to approximately 75–80%. Advanced is a physician-recommended option for specific clinical situations, not a default. All three are calculated from your body weight and hematocrit rather than a flat liter count, and all three are delivered to the same clinical standards — two nurses, protocol design by our board-certified nephrologist, pre-procedure lab review, post-session IV support, and ASFA monitoring. The difference is intent: Essential is built for wellness optimization, maintenance, and first-time TPE patients; Complete is the foundation of our initial reset series and intensive protocols, and remains our standard recommendation.

What is PlasmaRestore™ Advanced and who is it for?

Advanced is a 1.5× calculated plasma volume exchange — our maximum-clearance protocol. Clearance of plasma-resident substances isn't linear with volume: a single-volume exchange clears roughly 63%, and extending to 1.5 volumes raises that to approximately 75–80%. Advanced is not our default recommendation — Complete remains the standard for most protocols. Advanced is a physician-recommended option for specific situations where a higher clearance target is clinically appropriate: higher circulating burden, or protocols where your physician determines the additional clearance justifies the longer session and larger albumin volume. It's a consultation decision, not a menu upsell.

Is a 50% plasma volume exchange still effective?

Essential is a calculated protocol, not a flat-volume shortcut — the exchange is derived from your body weight and hematocrit, so a patient of any size receives the same proportional exchange. It is designed for wellness optimization and maintenance goals, and research suggests reducing circulating plasma burden may support those aims. For intensive therapeutic protocols — brain health, post-viral recovery, autoimmune optimization, or a higher toxic burden — Complete remains our recommendation, and your consultation is where that call gets made honestly.

Can I start with Essential and switch to Complete or Advanced?

Yes. Start with Essential. If you move to a Complete or Advanced series within 30 days, 100% of your first Essential treatment is credited toward it.

Why does Essential cost more than half of Complete?

Because most of what a plasma exchange costs doesn't scale with volume. Staffing, the apheresis equipment, physician oversight, and the majority of the disposables are fixed regardless of how much plasma is exchanged. The same logic runs the other direction — it's why Advanced costs less than 1.5× Complete. We price the procedure, not the liters.

How many sessions will I need?

Most patients begin with a series of 3–6 treatments, based on protocol. Initial series are typically built on Complete; many patients then move to an Essential Maintenance Membership (six treatments, roughly every other month) or Annual Membership (twelve treatments, monthly) to hold results. Advanced series are structured individually by our physician. The exact number depends on your health goals, current status, and how you respond to treatment. This is determined during your consultation and reviewed as treatment progresses.

How long does each session take?

A PlasmaRestore™ Essential treatment takes approximately 2 hours. A Complete treatment takes approximately 2–4 hours, and an Advanced treatment approximately 3–6 hours, depending on your plasma volume and the specific protocol. During this time, blood is continuously circulated through the apheresis machine. Post-session IV support is included in every PlasmaRestore™ protocol to support recovery before you leave the clinic.

Is TPE covered by insurance?

Avinity Health operates as a direct-pay clinic. TPE in a longevity or optimization context is not covered by insurance. Every package and membership price is published in full on our pricing and packages page, and our cost guide breaks down what's included and the questions to ask any clinic — both are linked from the pricing section above.

Is TPE safe?

Therapeutic plasma exchange has a well-established safety profile and has been performed in hospital and clinical settings for decades. Common side effects are mild and may include temporary lightheadedness or fatigue following the session. At Avinity, every session is overseen by trained RNs with a physician consultant available throughout. All candidates are medically screened before treatment begins.

Who is a good candidate for TPE?

Good candidates for therapeutic plasma exchange are generally healthy adults focused on longevity, chronic inflammation reduction, cognitive health, cardiovascular optimization, or post-viral recovery. Candidates should be in stable overall health, since the procedure involves exchanging a calculated portion of your plasma volume over a 2 to 6 hour session. Candidacy is confirmed during your consultation, where your health history, current medications, and baseline lab work are reviewed by our medical team before any treatment is scheduled.

What is the difference between plasmapheresis and therapeutic plasma exchange?

The terms are often used interchangeably. Plasmapheresis refers to the process of separating and removing plasma from the blood. Therapeutic Plasma Exchange (TPE) is the clinical application of that process — where the removed plasma is replaced with appropriate fluids (typically albumin) as part of a structured treatment protocol. At Avinity Health, TPE is delivered through the PlasmaRestore™ Protocol with guidance from an apheresis physician consultant with more than 20 years of therapeutic plasma exchange experience.

What replaces the plasma that is removed?

Removed plasma is replaced with clean replacement fluid — typically 5% albumin — as part of the exchange.

Do I need a central line or special IV access?

For most outpatient PlasmaRestore™ patients, access is through a standard IV line in each arm placed by the nursing team — no surgical catheter. One line connects to the apheresis machine; the other is a smaller line used to give calcium during the session. Access is assessed at your consultation.

Am I awake during treatment? What does it feel like?

Yes — you’re reclined and awake for the full session, and most patients watch TV or use their phone. After the two needle sticks that get you set up, you mostly don’t feel the exchange itself. Some patients notice brief tingling in the lips or fingers from the citrate anticoagulant, which the team addresses easily; the main challenge is sitting still.

Can I drive myself home?

Many patients can, but for a first session it’s smart to arrange a ride until you know how you personally respond.
