When testosterone is added from outside the body, the brain reduces the signals that tell the testicles to keep producing their own. Over time that can mean smaller testicular size, lower natural production, and reduced sperm count. For some men these effects are acceptable. hCG (human chorionic gonadotropin) mimics one of those signals. Prescribed alongside testosterone, it may help preserve testicular function and fertility potential while you are on TRT. It is not a guarantee. It is a small injection taken on a schedule set by your clinician and monitored with labs.

The most common cause is age-related decline. Testosterone drops roughly 1% per year after age 30. Other contributors include obesity, sleep apnea, chronic stress, certain medications, pituitary disorders, testicular injury, and genetic conditions. A lab panel identifies whether low T is the cause of symptoms and helps rule out secondary causes that may need separate treatment.
The most common symptoms are persistent fatigue, reduced libido, difficulty building or maintaining muscle, increased body fat (especially abdominal), brain fog, mood changes or irritability, poor sleep, and reduced bone density. Many men attribute these symptoms to stress or aging. Symptoms alone cannot diagnose low testosterone; blood testing is needed to determine whether it may be contributing.
